New Patient Registration

The Hopetoun Practice accepts patients from the area of Edinburgh bounded by:

  • to the NORTH, Ferry Road from Coburg Street to Inverleith Row
  • to the SOUTH, Waterloo Place and Regent Road
  • to the EAST, Easter Road, Duke Street and Great Junction Street
  • to the WEST, Inverleith Row, Rodney Street, Broughton Street and Leith Street

If you are within our catchment area and would like to register, you can download the required registration documents by clicking on the link below.  Please complete all sections of the documents to the best of your ability, and then bring them in to the Practice to finish the process.  Please also bring a form of identity (eg passport, identity card or driving licence) and something which shows proof of your address. 

Please do not send your registration documents by email or post, as you must attend in person for one of our Reception team to verify your identity and check your documents.  Registration documents that have been sent by email or post will not be processed.

Although it is not essential, new patients may book in with the Practice Nurse for a brief check-up at registration. 

Patients who are taking long-term,repeat medications should make arrangements to see one of our PRACTICE PHARMACISTS for review, before requesting prescriptions.

Registering as a Temporary Resident

If you are ill whilst away from home, or if you are not registered with a doctor but need to see one, you can receive emergency treatment from a GP practice as a Temporary Resident. A temporary registration can last for a maximum of 3 months. This will allow you to be on the local practice list, whilst still remaining a patient of your permanent GP.

To register as a temporary patient simply contact the local practice you wish to use. 

 

NB  Practices do not have to accept you as a temporary patient, although they do have an obligation to offer treatment in the event of a medical emergency.
