Change of Address
If you have moved to a new address, you will need to remain in our catchment area to stay registered with the Practice. You can check our catchment area on the following MAP, and if your new home remains within it, use the Change of Personal Details form below to notify us of your new address.
Change of Name
If your name has changed, use our Change of Personal Details form to notify us, but you will also need to come into Reception and bring your original marriage or civil partnership certificate, or your deed poll confirmation.
Change of Telephone Number or Email Address
Please notify us of this as soon as possible so that we have a means of contacting you if necessary. The simplest way is to use the Change of Personal Details form below.
