Let's Prevent Diabetes

If you are an adult with Pre-Diabetes, or otherwise at risk of developing Type 2 Diabetes, help is available to reduce that risk. You can join a group where friendly NHS staff, trained in Type 2 Diabetes prevention, will share practical, tried and tested advice.  You can find our more, and about how to refer yourself to the Let's Prevent Diabetes service by clicking HERE.

If you're an ethnic minority adult, and English isn't your first language, there are special groups where friendly, trained, bilingual NHS staff can share practical, tried and tested advice in your preferred language.  You can find out how to get involved in one of these groups by clicking HERE

If you've just been diagnosed with Type 2 Diabetes, there's a lot to take in, so there are a number of resources to help you:

             

Workbook for Newly Diagnosed Diabetes

This tells you everything you need to know about the condition so that you can understand it and start to manage it yourself.  Click HERE to download a copy.

        

Diabetes Education and Self-Management for Ongoing and Newly Diagnosed (DESMOND)

This is a 6-hour education programme designed to support you if you've recently been diagnosed. It consists of interactive, informal group sessions that are led by a team of trained facilitators. DESMOND groups may be accessed face to face at a site local to you, or virtually, via a free online platform as 3x 2 hour or 2x 3hr sessions.  

The programme includes:

  • Information on T2D, the causes and impact it has on the body
  • The chance to share thoughts and feelings around being diagnosed with T2D
  • Practical healthy lifestyle advice in order to manage your health and help avoid complications
  • Peer support and the opportunity to share experiences with others

The practice nurse will already have mentioned DESMOND to you, so if you'd like to sign up for a course, you can download the self-referral form HERE and send, or email it, to the address on the form. 

                 

Sick Day Rules for Type 2 Diabetic Patients
